My cousins experience:

Sadly, a cousin of mine died of pancreatic cancer early this year. But when he was diagnosed he had no job, no health insurance, too young for medicare. He negotiated with his doctors and hospitals, provided copies of his tax returns showing a low income. He got care - discounted, I believe the care he received was the best available - just didn't save him.

His medical bills came to about 1/15 or his estate.
