Well the illegal drug figures vary a lot. Take the low end estimates and cocaine and heroin may each (or even both combined) be lower than acetaminophen. Take the high end estimates and each might be 10 or more times as high. The high end estimates where probably overblown, but the exact number is still uncertain.

Some prescription drugs would also have to be on the list of candidates for deadliest drug in total (and obviously would be more dangerous than acetaminophen per use or per dose taken).

The real clear winner is IMO alcohol.

Nicotine is also bad, but it isn't the main killer from cigarettes, it causes the addiction but its the tar and toxins that kill you.
