Administrative costs are high due to the necessity of complying with Federal rules.

Well that, and also to contain costs based on having some idea of what they are really paying for.

Medicare and Medicaid, don't do much in terms of cost containment up front, that lowers there administrative costs but it causes two problems.

1 - It means they pay out for things they probably should not.

2 - In an attempt to combat number one, they can crack down harshly on doctors who don't know how to play the game right and mess up some code or do something unusual. Those who do know how to play the game can inflate costs and get away with it, while those who don't play the game well can be honest and face prosecution (I posted a story about that somewhere on SI, but I can't find it now).

why cant there by one standardized form for all companies?
