It might be all sorts of things. I'm not saying that that specific ranking is necessarily correct, I was just responding to your idea that no one was making such a claim.

While there are certainly major issues that aren't covered by that estimation (specifically quality of accident care, and any health issue that doesn't effect life expectancy), but at least what it does cover seems pretty straightforward and relevant, unlike say the WHO's ranking of "health care equality".
