There is plenty of blame to go around. Various members of congress from both parties, presidents since Carter, regulators and administrators at various government and quasi-government organizations, and a large number of private sector actors all deserve blame.

Yes, but lets keep the blame mostly in this century and let's have Republicans be the first to step up for a change.
