Someone with a high earned income (e.g. $500K) will almost always have a corresponding high unearned income (like I said, unless they're Shaq, who seems to spend every dime he earns, and more). So, their overall tax rate is often significantly lower than the example you cited.

Even people below $500K typically pay less than the full bracket. I saw that Palin had gross income of $166K and paid $25K in taxes - 15%, even though she was in a 28% bracket.

btw: Obama paid 33% on an income of >$4m. Bad tax planning...
