In baseball when is a runner/hitter awarded 3 bases?

If a player voluntarily runs out of bounds on punt coverage, there is a five yard penalty assessed. I don't believe it's a replay reviewable penalty (like 12 men on the field), but don't know for sure because it hardly ever happens.
