It shows up in the debt levels, and it shows up in spending bills, its just that not all of it is in the initial budget bill for each year. It hardly the only spending the works that way, and since it is accounted for and is in spending bills at some point, its not very meaningful to call it "off balance sheet". Its definitely "on balance sheet".
