I understand all that. My point is that a statement like:

is misleading. You can ask any doctor or nurse working in a hospital and they'll tell you that multiple problems is something they deal with continuously, 24 hours a day. Same for clinics - most older patients will have more than one disease.

WRT research, they are right. There is a good reason for lack of such research - it's hard enough to design a study in which one tries to isolate and study one or a few variables. It is probably simply not possible to isolate and study every possible combination of diseases and their treatments. Any ideas? In my estimation it can't be done. We'll need to continue relying on educated and trained humans - and their clinical judgement... :)
