"That falls into the category of ignorance and failure of self education or understanding one" This has some truth of it. But let's say 'annual preventive checkup is covered' and you asked the insurance for what is covered and you'll be asked, you give me the 'code' and I'll tell you if it is covered or not. For christ's sake, English (especially medical's is tough enough), do you think they should give you a list of 'codes' so I can decide which one I like to go thru.
