What other major form of commerce in the USA do you use where you the consumer don't have a clue as to the price of what you are going to buy until after you've made the purchase?

How about when a hard drive crashes in your medical facility's server and you're having to pay me to get your network running again? How about when your car is making a strange noise and you take it to the shop to get it fixed? How about when you're charged with a crime and you engage the Dream Team to defend you? How about when you take your taxes to your CPA to have him prepare your return?

I mean, really. There are many fields in which it is usual to rely on a professional to charge an appropriate amount for the services that are rendered.

Now, having designed and developed a number of medical billing and other A/R systems over the years, I can tell you that there are some fundamental differences between "typical" A/R and medical billing -- i.e., the assignment to third parties of the claims -- which I think is really what you're trying to get to. However, you do see similar relationships in many larger commercial A/R systems.
