I suppose, in underpopulated areas there is likely to occur some degree of trade-off between access and quality. In my group, I do (some) work with a nurse practitioner. She is a smart young lady and I think she is doing quite well. We do discuss her cases practically every day.

The "2%" story I read in JAMA, I think some time late last year or early this year.
