Meds are impossible to keep straight, even with a EMR.

Plus, documenting visits is enormously time consuming and difficult. If a doc puts it off for a couple of days which happens all the time he has forgotten a lot of important details.

I posted something the other day to that effect that no Doctor can rely on medical records for diagnosing and treatment


I review medical records every day in my job. They are uniformly a load of garbage. There are a few doctors that manage to turn out decent soap notes, but they are usually occ med docs that are used to turning out product for litigation.
