voice recognition software just doesn't cut it. Phone transcriptions are rapidly becoming a thing of the past.

It's happening. I know a Doc who started a new practice about a year ago and uses Dragon speech recognition combined, I think, with Amazing Charts. He is happy with the results. Also, in my hospital phone dictations are transcribed by a speech recognition program, and later edited when necessary. You get absurdities once in a while, but overall it works well.
