I've been watching for some years now with interest the "concierge" arrangements. Patients pay the doctors an annual retainer - anywhere from several hundred to several thousands dollars each year - for extra access and attention. In some cases Docs will accompany the patient on visits to specialists. Good for them! If they can afford it, why shouldn't they have it? Glad to see that so far the egalitarians have left these arrangements alone.

I saw something about a [sort of] similar arrangement a while back -- seems like in NJ -- where the doc received a monthly fee, sort of like an insurance premium, to cover patient visits and certain other items (maybe some lab or something). A fixed-fee arrangement to provide for some core services. The state board of insurance went after the guy. It seems like he ended up having to increase the fee to satisfy them.
