Just tell here she is going to die, unless she does certain things that are very very important. Look her in the eye, and keep the message short and heartfelt. It will get to her. Don't let them for a minute tell you they are "being careful", or "depend on their physician". Just look at them and in the eye, and let them know you know. Being polite to people who just don't get it isn't doing them any favours. That is, if you do know and are confident. If you aint' then re read every one of the posts made here, and on my own site, and re think it. I believe they are based on real clinical evidence. EC<:-}
