Regarding composites, I haven't studied them at all, so am speaking from total ignorance about them, but couldn't inspection be done by sticking on strain gauges and measuring deflection. If deflection under set loads is within tolerances, it should be okay. There are probably more modern ways of doing that than sticking on gauges - such as using lasers and computers to see exactly what the structure does under various test loads.

It surely isn't just a matter of building them then waiting until they fail while in operation.
