There are many people residing in institutions who fit the same description. I know Caroline has little memory of the 60s. She was just very young. I haven't seen the evidence that she is an "intellectual". Perhaps saying she is more intellectual than her brother was a real smack at him?

she certainly is more intellectual than her brother John was - but she lives in a bubble far removed from normal life

If Caroline was clueless asbout the real world and what she might face to go for the appointment, then she is pretty stupid. Intellectual smarts don't get one far without practical, real world smarts.
