Cosmetic and optional procedures should be second tier with higher co-pay and automatic second opinions (IMO).

I was rather imprecise in my wording - what I meant to convey was "doctors and hospitals would prefer to work without insurance documentation and filing issues". If they do the work and have established minimum eligibility requirement, they submit the bill to the payer. There might be questions and issues, but that is a single point of contact, not several as it is now.
