Adam Smith assumes rational, well-informed consumers.

When it comes to health care, the consumers are neither rational nor well-informed. The practice of bankrupting families in order to torture the elderly in their last few weeks of life is not rational, but it's what consumers demand in this country.

I'm not sure how we get around that.
