You said private enterprise. I was clearly referring to health insurance companies versus public entities like Medicare and Medicaid. Private companies will weigh the extent of the fraud against the cost to resolve it and take appropriate action. Government doesn't do that. They don't give a crap because it is government. When government uncovers fraud it is because they stumble onto it, for the most part. I have seen them act to stop Medicaid fraud where the up-coding was egregious. That was one incident in the last couple decades of working with this stuff.
