What do the private insurers do? The only thing I know they do is say "no" a lot ..... Medicare doesn't intervene that way

Medicare HMO's intervene plenty - pre-approvals, pre-certifications, you name it. "Regular" Medicare intervenes quite regularly by simply denying payment - AFTER the fact. With Medicaid, as a practitioner you're better off not to bill at all, just do the work pro bono -- the result is likely to be the same, but at least you'll feel better.
