They change their coverage and fee policies so often - and so arbitrarily - depending on whatever new smart idea they get, and what new vote they take - that for practical purposes the effect is almost indistinguishable from approving or denying claims at will.

I very much appreciate the point you are making about the difficulties of apparently abrupt policy changes. And the net result to the doctor's pocketbook and psyche may be the same as if they were acting arbitrarily on an individual patient basis. But conceptually, from a systems perspective, the two are very, very different.
