i can appreciate what you say.. wife used to get ingrown nails cut.. no problem for four years,, then all of sudden payments went nuts.. nurse or receptionist, had to made out two forms one for one foot and then other for the other foot. They did this for another four or five years then reimbursements were cut to something like 20/30% of cost of one foot if you did two feet.
