It depends on the PPO and the specialty and sometimes, even the procedure being performed.

For example, some PPOs don't require a referral for a screening colonoscopy while others do. Some don't require referrals to an Orthopaedist for setting a fracture but might for a surgery.

You can't correctly make a general statement about PPOs and referrals.
