In reading about this, it seems 68 is the age the mandatory counseling would start. Who decides - well, theoretically, the individual, but it seems we're moving in the direction of the government giving nudges in the interest of cost containment.

Who decides at the beginning of life who lives and dies, and who decides at the end of life if "now is the time" or when?
