I'm sure that's true in some cases, maybe even the majority cases. But while I was caring for my father, who lived in an apartment complex for seniors, I saw DNR's affixed to lots and lots of refrigerators and almost all the people I got to know had advance directives. As I said, my mother was an example of one who never accepted she was dying so I know that they exist. There's no way of knowing how prominent they are.
