It would unwise to make a blanket order like that years in advance when one is in reasonably good health.

It is each individuals choice to decide when to make an advance directive or assign power of attorney.

It is my understanding that Title 18 of the current Social Security Act already requires that you are to be provided with written information concerning your rights to accept or refuse medical treatment and your right (not your obligation) to formulate an advance directive.
