They don't work "on commission". Where did you get that idea?

The entire concept behind fee-for-service is that the physician is paid for the work he/she does.

Some physicians may choose to work on salary for whatever reason. But you're going to have some pretty major problems if you try to force that concept on the medical community.

The complexity of determining fair compensation for physicians today simply cannot be overstated, even in a small medical group.

But the bottom line is that if you cut payments from MC/MA to physicians any further, you are going to see some fairly major problems in finding physicians to see the patients who are covered by these programs. And I really can't think of any other reason Schumer would be talking about this.
