Agreed. If you look at perhaps the two best nonprofits from either extreme of the "per patient expense spectrum" you'd have Kaiser on one side and Mayo on the other. And they share a surprsing number of characteristics (and I'd say their practices are converging over time). Probably the main difference is in their readiness to adopt new (unproven?) technologies, Mayo is much more likely to do that. And of course Mayo has a massive endowment and massive ongoing research program. The solution to maximizing Quality:Cost lies in finding what these two have in common, IMO.....
