What an experience you had!

In Virginia in the 1970's------we had not decided on a name for our daughter so the doctor and hospital did a handwritten report for us and one for the State (not the County) and noted that we could let the state know her name later with the spelling exactly as we wanted.

So we have the official paper copy of the doctor/hospital document and a copy of the same from the State. We notified the state of the spelling of the name, a year later we received a piece of plastic like a credit card that gives the basic details with a social security number.
