The documents are photostats, i.e. copies of the originals. It looks like the Health Dept combined two separate pieces to make the certified copy. You can clearly see that the two sections are slightly offset. The top 3/5 is the original, and the bottom 2/5 starting with "THIS CERTIFIES THAT THE ABOVE IS A TRUED AND CORRECT COPY..." is the certification of the original. I think the mother requested these copies in 1966... Unveiled!
