I'm curious if you can shed any light on how doctors and surgeons determine rates. Say for example something like having tonsils removed. Does the average surgeon bill based on what others around him charge? In addition, the Republicans are big on consumers having skin in the game, the theory being they will be more careful about using healthcare. How does one determine in today's world what is a reasonable charge for a procedure or test, visit, etc....
