Very good point. If people embrace a healthier life style, perhaps there could be a small temporary decrease in costs, but not for long. Healthy lifestyle doesn't mean that people will stay healthy until the day they die. They would merely start getting sick on the average a little later - and probably stay alive - and sick - for longer periods of time.

Large older population equals high costs. There is no way around that. Unless, of course, we become really heavy handed with rationing... which runs contrary to American culture and tradition.

Dunno. Probably not. After all, we all die of something. If not lung cancer then something else. And dying later because we didn't die of lung cancer early may make health care costs go up because we use the system over a longer period of time.
