How do I know? I've recieved the training! Part of my management training (at a software company) was evaluating my employees costs to our health insurance (my company was self insured) and determining whether they were worth keeping on the payroll. If they got too expensively sick, I was to find some way to get them to quit or to give them the boot, other than the health care costs!

Sounds like a problem with the company you worked for. Nothing to do with how insurance claims are processed.

As to the procedure where you worked, while I'm no attorney, I believe what you were instructed to do would be a violation of ERISA. Which, once again, has nothing at all to do with the ways in which employee health care claims are processed by health insurance companies.
