I have reservations about using quality adjustment in health-care cost-effectiveness analysis. I think that life years makes sense. But when you adjust for quality other than at the most basic level, like vegetable or death-row inmate, dunno. I'm uncomfortable with the notion that some lives are officially more worthy than others.

As for the Markov math, I will just pull the covers over my head...
