BTW, that was made possible by one of Ted Kennedy's legislative accomplishments: the Health Maintenance Organization Act of 1973, which created the HMO by legalizing capitation. Prior to the HMA, it was illegal in nearly every state (all except California, if you want a hint as to where I'm going) for a doctor to accept compensation for not treating a patient. Prior to the HMA, health insurance was exclusively fee-for-service.
