This, of course, is a liberal lie.

If an insurance company denied a claim for a treatment that their contract called for they would be sued to within an inch of their existence.

The incidence of this is beyond rare. In those few instances where it does occur, the Left has made a big deal of it. But it is not usual or typical or frequently occurring.
