i thought accepting all conditions was already a given by insurance. i also believe some type of cap for extreme costs has been resolved so no person has to go broke.

Unless that cap is $50 I don't see how it insures "nobody" has to go broke. I know of people right now who don't have $50 but can find money to eat at Wendys and drive nice cars. They're broke all the time.
