Many mistakes are made... for example, how come the SEC ignored notices about bernie madoff? In order to make a decision on anything, you have to decide what to consider worthy... and even then many mistakes are made... no one is perfect... and considering the admission from sadam himself that he wanted to make it appear as if he had WMD's hidden, there was little room for giving anyone the benefit of the doubt...
