We only operate out of two states and I have no idea.

My employer is a huge one. In different areas of the country you can get regional insurance that is only offered there, mostly HMOs, I think. But if you go with the big providers, like Blue Cross, which I have, you're good to go no matter where or how often you are transferred. I don't see the benefit of making individual buyers undergo a change whenever they move.
