Certain things that would be regulated are obvious and many/most insurance companies would do them anyway. For example, car insurance policies have mechanisms for inspecting damage to vehicles. That's SOP. There is no need to regulate them because they would happen anyway. They would go into the standard/national policy. You would want to have some oversight to make sure that happens. States would do a fine job of that. Media could play a role. Or you could have some congressional committee authorize a review.

Then the insurance companies would add to that various combinations of features to address different markets, for example high and low deductibles. They would add other features that might attract customers, for example, the way some car insurance touts how easy it is to get an estimate and they even come to your car's location.

Then the states would tack on any fillips that they saw fit to regulate for whatever reason or non-reason. If they found the national policies inadequate, they could add whatever they want. That would put pressure on the national policies to be adequate.

Interstate health insurance sales only seem odd to you because you're used to doing it that way. But most products are sold nationally. California tends to make up its own rules for all sorts of things but mostly states stay out of most things. But when you think about it there's no reason in the world to have different insurance rules in Nebraska than in Wyoming.
