I'm aware of that - carelessness that a good editor would have caught. Most authors are careless because they are focused on the subject and not the details. Very unfortunate in his case.

But there are other occasions.

I had a writer who "borrowed" some passages and tried to work them into his MS. Very obvious when the style changes - especially when Dickens - LOL - I pointed it out to him and suggested that he find another editor.
