Mostly American consumers because many countries restrict the ability of drug companies to recoup the cost of research from sales in that country.

Actually it's mostly US taxpayers... government and institutions fund most basic research... drug companies next to none. Socialism Peter, socialism....

If you think about it, it's no different from any other trade restriction, practiced by nations with "Single Player" (no typo) healthcare systems. In effect, they say - "I get mine at production cost, and screw everyone else".
