Dealing with third parties involves submitting bills, dealing with denials, "pre- authorizations"... it's a big thing, it requires an entire front office, including at least some skillful people who know how to get paid at the end of the rainbow.

Only dealing with and billing for co-payments is probably more work than a simple pay-as-you-go arrangement.

I like the idea of larger deductibles. My next insurance will have a 2K per person and 4K per couple deductible. I would take higher, but this is the only one offered.
