Sell the organs to the highest bidders = that solves that problem: <Interesting idea, and silly as it sounds some people believe they will take on traits of their donar and would refuse organs from maniacs. I'm sure they'd be overruled by the know it all police and if they still refused, their case would be turned over to the death panels. >

Some would refuse on ethical grounds. Some would refuse on religious grounds [swapping blood]. Some would refuse because they don't like surgery. Some would refuse because they like to refuse. Each to their own.

Plenty of doctors would also refuse to do the work.

Doctors in India or China or Thailand or Mexico or somewhere would probably be willing to take a broader ethical view. Religious edicts preclude a lot of genetic and other "Messing with -od's plan".
