What happens when they go and ask for a check-up Hugh? What happens when a diabetic shows up EVERY FEW HOURS, EVERY DAY needing insulin? Does the indigent cancer patient get a private room?

They will do a check up, they will supply the insulin. You don't need a private room to be well treated.
