Switzerland has a far more no nonsense approach.

If you drop your health insurance coverage in Switzerland you go straight to a court hearing and then to jail if your refuse to reinstate your health insurance.

All this occurs even before your medical insurance lapses!

If monetary penalties for refusing to maintain medical insurance are not effective, the Swiss approach may become attractive - especially to the insured and the taxpayer.

You might not be able to legislate morality, but legislating mandatory health insurance is very easy to achieve.
