Improvements for any person or group of people should either not effect the score (if their insignificant), or they should improve it. I'm confused... you are talking about trend not the fixed "grade" for a country. I think WHO was taking a photograph, not looking at a trend. Also things can get worse; more people can become uninsured or have to revert to much worse policies (underinsured). Again we may very well have the VERY BEST health system in the world for a very limited number of people.
