You cannot choose to be uninsured for health purposes -- somebody is going to pay the tab sooner or later, and in addition to being a freeloader you get third rate care and clog up the health care system.

There is no kind of insurance other than socialized insurance -- insurance is, by definition a socialized approach to risk management. And when it comes to health care you can pay up front and receive reasonable care, or you can pay twice as much later and receive crap. We overpay and dish out crap because we are too immature politically to figure out this simple fact of life. To say that government has no role is to be unprepared to think this through beyond ideological soundbites.

From what I understand, France has such a system, but the insurance companies are strictly regulated and offer very similar polices. There is a "government option" for total welfare cases.

The WHO rates their system the best in the world, but doctors feel underpaid. But, they still make house calls!
